Insurance Plans Accepted
For your convenience, we participate in a large number of health insurance plans. Please bring your insurance cards with you at the time of your visit. Also, many insurance companies and HMO’s require the patient to obtain a written referral from a Primary Physician. Each patient is responsible for obtaining the proper referral from his/her doctor. You may bring referrals with you at the time of your visit or have them faxed directly to our office at (845) 691-7589. If you don’t see your insurance company listed in the list to the right, please call the office as changes and updates to our accepted insurance plans are made periodically.
